[bisq-network/bisq-docs] Update manual payout doc (#44)

These updates were all made in the context of issuing three back-to-back manual payouts to a pair of traders. No idea why these trades could not be closed normally; have never seen multiple incidences like this.

 - Put ToC on the left so users can see next steps even while scrolling
 - Add a handy template for all the fields that need to be filled out near the top of the doc
 - Update the now-outdated screenshot and note around pre-filled fields in the payout form
