Yes that idea of micro payent channels is mentioned in my overview and Coinffeine was a project implementing it, but problem for Fiat is that there are not many payment providers which offer API access so that the app can do automated transfers and which are very cheap/free. OKPay was used by Coinffeine but then OKPay is basically the exchange and can track it's users trade behaviour. If there would be a standard for all banks and all would offer API access then it would be an interesting model for Fiat. But even then having many small transfers to/from the same person would be very easy to spot as such a BTC trade.
For Altcoins it would work by using off-chain mirco payments which increment each round. The incremental step at a round is the remaining risk but it can be set pretty small or secured by some collateral model. I am not aware that anyone has implemented it for a DEX.
